Emergency contraception

Questionnaire/history: History from? Date of birth confirmed? Reassured that consultation will remain confidential? Date and time of unprotected sexual intercourse (UPSI)? - Within < 72 hours? - Within < 120 hours? Mode of UPSI? - Sexual abuse, rape or non-consensual sex? - Absence of contraception? - Failure of Barrier Method? - Pill error? - Reduced pill efficacy due to vomiting or diarrhoea? - Reduced pill efficacy due to concurrent medication? - Vomiting within 3 hours of taking emergency hormonal contraception? - Other? Risk for sexually transmitted infection? Exclusion of prior pregnancy? - Last period later than the expected time? - Last period lighter or shorter than normal? - Last period unusual in any way? - Has UPSI taken place since last period? First day of last period? Usual cycle lengths? Expected date of ovulation? Breastfeeding? Other UPSIs in the last cycle? Past medical history? - Liver disease? - Malabsorption? - Porphyria? - History of unexplained/unusual vaginal bleeding? Current medications? Drug allergies? Examination: Weight? Height? BMI? If BMI > 26 kg/m2 or > 70 kg: ulipristal acetate or a double dose of levonorgestrel [unlicensed indication] Management: Indication for emergency contraception confirmed and method chosen according to NICE CKS and FSRH? Information about levonorgestrel given according to NICE CKS? Information about ulipristal acetate given according to NICE CKS? Advised: - To call back if vomiting occurred within 3 hours or diarrhoea for more than 24 hours after taking pill or lower abdominal pain - Possibility of early/late/light menstruation - To contact own GP if period is delayed by more than 7 days or period is abnormally light, heavy or painful - Pregnancy test 3 weeks after last UPSI Offered options for testing for sexual transmitted diseases including HIV Offered regular method of contraception Reference(s): FSRH (2020): FSRH Clinical Guideline.
