Choking in children
Encouraged child to continue to cough?
As coughing became ineffective shouted for help and gave 5 back blows with below technique?
<= 1 year:
- Supported the infant in a head-downwards across my lap in prone position
- Supported the infant’s head by placing the thumb of one hand at the angle of the lower jaw, and one or two fingers from the same hand at the same point on the other side of the jaw (as demonstrated here)
- Delivered up to 5 sharp back blows with the heel of one hand in the middle of the back between the shoulder blades
> 1 year:
- Child is positioned head down
- As small child placed across my lap or supported the child in a forward-leaning position and delivered the back blows from behind
As back blows did not relieve obstruction gave up to 5 chest thrusts or abdominal thrusts with below technique?
<= 1 year: chest thrusts
- Turned infant into a head-downwards supine position by placing my free arm along the infant’s back and encircling the occiput with my hand
- Supported infant down my arm, which is placed down (or across) my thigh
- Identified the landmark for chest compression (lower sternum approximately a finger’s breadth above the xiphisternum)
- Delivered up to 5 chest thrusts (similar to chest compressions, but sharper in nature and delivered at a slower rate)
> 1 year: abdominal thrusts
- Clenched my fist and placed it between the umbilicus and xiphisternum, grasped my fist with my other hand and pulled sharply inwards and upwards, repeated up to 4 more times
- Ensured that pressure is not applied to the xiphoid process or the lower rib cage as this may cause abdominal trauma
As obstruction was not dislodged, called/sent for help if it had not arrived and repeat this sequence of alternating up to 5 back blows then up to 5 abdominal/chest thrusts until the airway is clear?
As child stopped breathing, lied on their back, opened the airway and if the obstruction could be seen, attempted to remove it with a single finger sweep, did not attempt blind or repeated finger sweeps, gave 5 rescue breaths and proceeded to CPR?
